Thanks ! This is very useful

Hi..Why is this affidavit required?
I think Sakhshi we have to give this document once we get call from school and admission is confirmed?

Gyan Bharti School is asking for Self Attested Affidavit in case of Single Child.

Can anyone help me out?

Affidavit is usually self attested declaration given by "Deponent"/self.

The attestation is usually done by a notary or a maigstrate (depends on ur requirement), however that attestation doesn't mean anything, since they are just confirming that they have read what you are declaring. In no way the responsibility of your declaration is with them.

So @ biswajit, in your case just get the affidavit made and get it signed from any notary.

Thanks a lot Shival ji for this clarification and the affidavit format.
thanks shival for correct information..sevaral schools have asked for first child affidavit.
