If school has not asked for any specific format, you can make common affidavit n its school specific as some schools accept photocopies also but not all at the time of applying.

Its Notary which signs the affidavit for most of the schools(if not specified)

If any school has specified to be signed by SDM, ask any of of local Notary he will get the needful.This the the way parents have been doing in past.

Which schools are asking for SDM affidavit  in your area ?

AT THE TIME OF ADMISSION WE NEED TO SUBMIT THE ORIGINAL AFFIDAVITE.

Hello,

If school not mentioning first child affidavit in list of document required then also do we need to submit affidavit or it is not required?.

Only schools which gives points  for first child ask for Affidavit. else no need.

My wife got our first child and girl child affidavits stamped through her office in Noida and as a result they are on UP Stamp paper. Is this a problem for schools who have asked for notarized affidavits?
