Hello,
I live in Patel Nagar and was considering Modern School as one of the options for my daughter, who will be turning three in March 2015 (DOB: 16-03-2012). I just wanted to confirm whether Modern School at Barakhamba Road offer Nursery classes. If yes, when will the admission process be starting. Thanks in advance.

Downloaded form must be submitted along with necessary documents from Wednesday 31st December 2014 to Tuesday 13^th January 2015 on all working days from 9:00a.m. to 12:00noon

Seats for EWS candidates are reserved as per Government directives. Forms for EWS category can be collected and submitted from school between 9.00 a.m to 12.00 noon on all working days from 30^th December 2014 to 13^th January 2015.

Prerequisites: 

Ensure that you have the following before you begin to fill the form 

• Pop-up Blocker of your Browser is off.

• For Internet Explorer : - Go to Tools> Pop-up Blocker >Click on Turn Off Pop-up Blocker

• For Mozilla:- Go to Tools > Options > Content > Uncheck the “Block Pop-up Windows” > Click on OK

• For Google Chrome:- Go to Menu > Settings > Show advance Settings > Privacy > Content Settings > Pop-ups > Select “Allow all sites to show pop-ups”.

• Before you register online please read all instructions carefully and click on the Proceed Link given at Bottom.

GUIDELINES FOR FILLING ONLINE REGISTRATION FORM FOR PRE SCHOOL (NURSERY)
 All fields marked * on Registration form are compulsory.
 An incomplete registration form will automatically be rejected.
 After completing the registration form, click on SUBMIT to upload the form. You will get
an auto Generated acknowledgement receipt with a user ID and password on it. These are
to be used for any future references.
 Download the filled out form along with the acknowledgement receipt. Affix the latest
passport size photographs in the given spaces in the form.
 Attach photocopies of required documents (mentioned below) and acknowledgement receipt
to the downloaded copy of the form and submit this to the school before Tuesday 13th
January 2015 till 12 noon with Rs.25 for registration.
1. Date of Birth Certificate of the Child (Affidavit will not be accepted)
2. Proof of Residence: (any one)
i. Ration Card
ii. Driving License
iii.Voter I-Card
iv. Passport
v. Aadhaar Card
vi. Latest Paid Electricity Bill / Latest Paid Water Bill / Latest MTNL Bill
vii. Registered Lease Deed in case of rental accommodation.
Minimum period of residence at given address should be of PAST ONE YEAR. Points will not be
awarded if documents are insufficient to prove residence at the mentioned address for the PAST
ONE YEAR. If the document submitted is older than one year, please submit an additional
document i.e. Telephone/Electricity/Water bill or Rent agreement in the name of any one PARENT
which proves residence at the mentioned address between December 2013 and March 2014.

3. Proof of Alumni (passing certificate copy or mark sheet copy of class XII)
4. Certificate of SC / ST / OBC
5. Supporting documents for the Single Parent
6. Proof of Sibling (Only Real brother or sister studying in this school) Attach: Identity Card/Latest
Fee Bill of the sibling issued by the school office for 2014-15
7. Proof of first child (Affidavit)
8. One passport size Photograph of child.
9. Medical certificate and records of the child (only for children with special needs).
You are requested to declare correct medical status vis-a-vis special needs, if any, in the application
form. This has to be accompanied by a complete medical report from a CBSE approved Doctor /
Institute. Failure to declare correct medical status will automatically disqualify the child EVEN IF
ADMISSION HAS BEEN GRANTED.
10. If you are the Guardian please enclose all the following:
i. Authority letter by the parent (s) or an affidavit.
ii.Your recent photograph.
iii.Proof of residence (please see point 2 above)
NOTE:
 Verification of submitted documents will NOT be done at the time of submission of
registration form.
 Registration is no guarantee of admission.

• A print out of the registration form, with a passport size photograph of the applicant pasted as indicated. It is mandatory for both parents to sign the form (other than in the case of single parent). Sibling Applicants: The admission order/ latest fee bill of the sibling studying in DPS International only will be accepted. Embassy Officials: An attested photocopy of the ID card, Passport & Visa of applicant and parent/s Global Transfer Cases: Proof of overseas transfer within the period making the applicant eligible for admission i.e. (between 31st  March 2012 – 2015 with a minimum of 1 year posting overseas) Multi National Company Employee: Certificate from company stating that they have offices operating in foreign countries (to specify the names) and that the employee may be transferred to any of the offices listed. First born: Affidavit stating the same. Original Birth Certificate Foreign Nationals : An attested photocopy of assport and visa/ OCI/PIO card of applicant and parents. Single Parent: An attested copy of the custody papers in case of divorcees.
