This Registration Form is for Pre-School class only. Please do not use it for any other class.
The Registration forms will be available on our website www: montfortschooldelhi.in from 29th December from 9.00 a.m to 15th January 2015 till midnight.
The Registration Form is to be submitted in the school office from 12th January to 20th January between 9.00 A.M. to 12.00 noon on payment of Rs.25/- An incomplete Registration Form will be discarded.Handwritten registration form will not be accepted.
The children born between 1.4.2011 and 31.3.2012 are eligible for registration for the academic year 2015-16.
The Date of Birth Certificate in original from Municipal Corporation / Panchayat only will be accepted at the time of admission. No other document will be accepted for the purpose.
One recent passport size photograph of the child and one 2b size photo of the family will be required to be attached on the Registration Form. Please paste the family photo on the registration form.
It will be the responsibility of the parents to see that the Form is complete in all respects. TheRegistration Form if incomplete in any aspect will be rejected.
The parents are responsible for the correctness of data entered in the Registration Form.Registration and even admission will be cancelled if any information recorded therein is found to be false.
Please note that the school does not provide any transport facility for the students. Those admitted will have to make their own arrangements.
The list of eligible students will be displayed on 2nd March 2015 on the school notice board at 4.00 p.m and subsequently on the website. (www.montfortschooldelhi.in)
The list of the selected candidates for admission to PRE-SCHOOL will be displayed on 9th March, 2015 on the school notice board at 4.00 p.m. and subsequently on the website.www.montfortschooldelhi.in
The parents are required to attach a self attested photocopy of their ward’s Date of Birth Certificate. Affidavit in place of Birth certificate will not be accepted.
Please attach a self attested photocopy of any one Proof of residence with the Registration Form, through:
- Election ID Card/Passport/Aadhar Card/MTNL Land Line Phone Bill
- Electricity Bill/Water Bill.
- N.B.: Rent Agreement will not be accepted.
The parents are also required to produce original copies of the documents when they come for the Admission of the child, once the selection for admission is confirmed by school.
All documents submitted at any time will become the property of the school and will not be returned.
All selections and admissions are done on the basis of the information given in the Registration Form and the documents submitted along with it. If, at any stage, the information is found to be incorrect, the admission of the child will be cancelled. The school reserves the right to reject Registration Form with either incomplete and/or forged documents.
Any grievances with regard to admission must be given in writing from 3rd March 15 to 6th March 15 between 9.00 a.m to 12.00 Noon. A reply will be given within 7 days of reviewing the grievance.

  - (20 % seats would be reserved for the School Management. It is meant for the people who in the discretion of the Management have worked for the development of the school in various capacities over long years, wards of displaced person or affected in other ways)

The Montfort Senior Secondary School is a Catholic minority institution run by the Brothers of St. Gabriel and it has preferential obligation to students of the Christian community, for admission.
Any recommendation, use of influence(direct of indirect) offer of donation, etc. will lead to cancellation of the registration/admission of the child.

WARNING :
In the interest of the public it is informed that Montfort School does not accept any donation in kind or cash. It has come to our notice that certain persons ‘pose’ as agents and promise to secure admission on payment of money. If the child gets admission on his/her own merit, the so-called ‘agent’ takes the credit (and money); if not he expresses his helplessness, SO-PLEASE BE ON YOUR GUARD.

Points will be based on Distance, Single parent, Siblings, Alumni, Roman Catholic and will be calculated automatically from the registration form.

Date of Birth should be between 1st APRIL 2011 AND 31st MARCH 2012
Incomplete or incorrect application will be rejected.
Tick within the appropriate box.
All the fields must be treated as mandatory(i.e. try filling up as much information as possible), though you may omit the information that may not be applicable.
CAUTION: DO NOT FILL MORE THAN 1 FORM, ELSE THE CHILD'S CANDIDATURE WILL BE CANCELLED.
Scanned copy of Recent Photographs of Student and Parents (in JPG, PNG, GIF format) must be kept before filling the form. The dimensions should be 120px (Width) and 150px (Height). If softcopy of photograph is not available, parents are requested to kindly affix Recent Passport Size photographs on the Printout of Receipt before submitting the same.
In case any document (as mentioned below) is found false, the admission is liable to be rejected.

The following documents are to be submitted:

Attested Certificate of Birth, issued by the Municipal Corporation.
Medical Fitness Certificate of child by registered medical practitioner.
Proof of Residence
Identity Proof of Parents/Guardian.
Proof of Guardianship (If applicable).

In Sibling cases, last fee recepit must be enclosed.
Any other documents validating data entered in the form.
Four copies of Passport size Photograph of Student.

Instructions for filling the Registration Form

	Children whose birth date falls born on or between 1^st October 2010 and 31^st March 2012 are eligible for registration for the academic session 2015-2016. You may apply online through the website OR submit the offline form which is available at the school campus. *It would be preferable that the online form be filled.*
	Important: You are advised to fill in only one form - either Online OR Offline, to avoid cancellation of both the forms.
	The online and offline registration forms will be available from 10:00 hours on 30^th December 2014 to 14^th January 2015. The last date for receiving all forms is 1600 hours on 14^th January 2015.
	The admission process has been designed to provide equal opportunity to all and to maintain transparency in the admission process. The criteria for admission for open seats are as follows:

Guidelines for filling Online Forms

This is an electronic registration form for Pravesh Vatika. Please read the instructions carefully before filling in the form to avoid the rejection of your form.
Each form will be issued a unique Registration Number after the acceptance of the form. Kindly take a print out of the Receipt Page. This serves as a record to know the status of your application and for any further correspondence.
The registration form will refresh while it retrieves dynamic data from our database.
Do not press the ENTER KEY to move from one field to another. Use the TAB KEY or the mouse control for the same.
After completing the application form kindly click on the SUBMIT FORM button.
To avoid rejection, DO NOT fill more than one form for one applicant.
All fields are mandatory. Incomplete forms will not be accepted by the system.
Please enter your email id correctly as you will receive further communication on the address entered.
When user select values in some List box & radio buttons, the registration form will refresh for retrieving the data from the database. Till refreshing completed user should not enter or select any data in the registration form as it may lead to loss of entered /selected data.
Please note that when you have slow internet connection the registration form appearance, refreshing of the form and submission of completed forms may take some time, please be patient.
The areas/localities/colonies within range for criteria of selection are built in to the system. If you do not find your locality, you may choose the 'Others' category. You will then be able to enter details and the distance from the school campus.

Final admission is subject to the verification of the following documents [in original]:

Birth Certificate issued by the Municipal Corporation
Proof of residential address [Passport/ Lease Deed/ Electricity Bill/ MTNL Telephone Bill/ Electoral Card/Aadhar Card]
Note: School will also verify the residential address through home visits.
In case of sibling applicants: the identity card/admission no of sibling studying in The Shri Ram School whether at the Junior School i.e. Vasant Vihar, the Senior School i.e. DLF Phase III or at The Shri Ram School, Aravali
In case of adopted: Adoption Deed.
For proof of first born child: affidavit affirming that the child seeking admission is the first born child.

In case of twins, if one of the twin children gets selected, then only will the other be considered under sibling category at a later stage and not during the submission of the form. Note:The documents will be verified at the time the admission fee is deposited.

Registration Form for admission under Freeship-Disadvantaged Group and Weaker Sections - Economically Weaker Section, Children with Special Needs,SC/ST/OBC, Orphan

Parents are requested to fill in the Common Admission Form (as prescribed by the Directorate of Education) available at the school campus free of cost

Documents required for admission on free seats :

For the admission of child belonging to weaker sections:
a. Income certificate issued by a revenue officer not below the rank of Tehsildar
b. BPL ration card (yellow coloured)
c. AAY Ration Card (pink coloured)
For the admission of child belonging to disadvantaged group:
a. A certificate issued by a revenue officer not below the rank of Tehsildar or any other competent authority
b. Caste Certificate by DC Office
c. Medical Certificate from government hospital or a report of diagnosis from a recognised hospital. (In case of Special Need or/ disability)
d. A Certificate issued in accordance with instruction issued by the Department of Women and Child Development, GNCT of Delhi, vide order dated 11.12.12 in case of Orphan Child.
Proof of residence:
a. Ration card issued to the parents (mother/father having the name of the child).
b. Domicile certificate of child or of his/her parents.
c. Voter I card of any of the parents.
d. Electricity bill/MTNL bill/Water Bill.
e. Unique Identity card (Aadhar) in the name of Father/Mother/Child issued by the Government of India.
f. Passport in the name of any of the parents or child.
