General Instructions for online registration:
A. Parents kindly scan the following documents before starting the online registration :
(a) Parents’ photograph with the child.
(b) Child’s photograph
(c) Date of Birth certificate with the name of the Child.
(d) Valid residence proof of address : (any one)
i) Ration card issued in the name of parents (Mother/Father having name of child)
ii) Domicile certificate of child or of his/her parents.
iii) Voter l-Card (EPIC) of any or the parents.
iv) Electricity bill/MTNL telephone bill/Water bill/Passport in the name of any of the parents or child.
v) Aadhaar card/UID card issued in the name of any of the parents/ Child.
B. An affidavit on the non-judicial stamp paper of Rs.10/- duly attested by notary public or oath
commissioner or the magistrate, declaring :
1. The distance of your place of residence from the Queen Mary’s School, Model Town.
2. An undertaking that in event of the distance stated in the affidavit found to be incorrect at
any stage, the admission shall stand cancelled automatically.

C. In case of Christian Candidate (Mandatory documents)
a) Letter from Presbyter-in-Charge confirming the membership of the Church.
b) Baptism Certificate of the Child.
c) Confirmation certificate of the parents.
D. Sibling proof: Photocopy of latest I-Card of the real (biological) sister studying in the school.

Is the distance affidavit required at the time of online submission? I checked the online form and it doesn't ask for scan of the affidavit. Also, they are charging Rs 25 for offline form submission but Rs 100 for online submission. Isn't this illegal as per DoE's guidelines?

No Distance affidavit not required unless asked

Rest yes it is illlegal

Thanks! Any idea about the quality of online documents to be attached? Is it ok to attach photos clicked through a good mobile or are scans compulsory?
