Immunization chart is in the child's birth record. Medical Fitness certificate you should get from Govt. registered doctor or child's paediatrician in their letter head.

Hi Shalini,

I am a last year parent and for immunization chart just take a photocopy of the all the vaccination done for your child.

For fitness certificate you ask your child's pediatric doctor to make one stating that he / she has examine your child and the child is medically fit to attend the school.

In my last year experience they take photocopies.

Does anybody know how the management quota seats work? There is no information on the different school website as for how the management seats will be distributed. Kindly help

• Sibling proof if applicable
