Is there any registration form needs to be carried while verification. As we are in receipt of registration number only. Please advise.

just the original of address proof and birth certificate of child

Just carry the original of documents that you have mentioned on the form... it will just take 15-20 mins total time (Including the waiting time)

Are they checking the the additional documents also the sports certificates etc
