manishji,

you can get affidavit form any notary and get it attested to sdm office in defence colony(behind frank anthony school). both parents have to be present at sdm office for attesting of affidavit.

while filling the form of frank anthony you had submitted the affidavit attested by sdm , you can follow that process as well.
