make a affidavit on 10/- paper from any notary public mentioning the first child, notary people do hv the format fot the same

JOINT DECLARATION OF FIRST CHILD BY BOTH THE PARENTS

Father and Mother can fill up and sign as below format for a Joint declaration as the case may be

We  parents  __________name______________ (Father) and ________name _______________________

(mother) R/o__________________________________________________________

do hereby solemnly affirm and declare as under :

1. That I have applied to Indian School., New Delhi for admission of my

daughter /son named _______________________________ in Pre-School/ Nursery

(2015-2016).

2. That I declare that ____name______________ is our first born(Boy /Girl) child /

and he/she does not have any elder sibling.
