Hi- does the print out of the completed online form have to be submitted with the school right now or only at the time of verification of documents?

Hi, In DPS Mathura Road Delhi, asking to submit medical certificate by doctor. Is Vaccination routine  book will only not be sufficient?

Vaccination routine book will not be sufficient for the purpose of verification of documents by the school.

The website has a draft of the Medical Form which should be filled and signed by a medical practitioner. Please note that this form is one of the documents required during document verification and therefore is a must and should not be missed.

I have filled DPS Mathura Road's online form without this Medical form, and did not find any option to upload medical form online.

I believe at the time of admission we need to submit these documents, correct me if am wrong.

Dps mathura road is asking to submit a dd of rs 55000. Is this amount refundable in case of no admission?
