You should fill her name as mentioned in the birth certificate. If you wish to add surname, then get the name in birth certificate rectified first.

Hi sanjeev, thanks. But it will take time to change name on Birth certificate from mcd. So can I change the name after admission on school as well as birth certificate?

I think moderators/administrator can guide better here. I think you can change name after but it is a long and tedious process.

@Mohit. it will only matter if the school ask for the birth certificate with chils name as most of school dont demand but few demands. If they demand then it will take 1-2 days for that only. Dont worry. It will also depends on the school whether they can manger with it.

But will it affect the admission allotment if they find the certificate and form name different, can they marked that type of farms under wrong details filled categories.

@Mohit. it will only effect if the school asks for name printed over the certificate otherwise no problem
