Various schools are asking for medical fitness certificate.

Do i need to contact the specific school for the format required for medical fitness certificate or its a standard format for all the schools.

Can someone please share the format of medical fitness certificate required for school admission.

Do we need to get a print of the same on letter head of hospital or we just need signature from the hospital/doctor on a simple printout?

Hi Nidhi,

The doctor has to write on the letterhead to mention that the child is completely fit and she/he does not have any disease. We need to submit the original copy with doctor's  signature.

Please note that print of screenshot of google map showing distance between the school and residence is required while submitting the form in Adarsh Public School, C-Block , Vikas Puri.
