Take Photocopies of Birth Certificate, Address Proof, First Child Affidavit & Medical Certificate of child along with all originals of the same for there verification. Also they will require original medical certificate and the process was quick.

No need to submit original medical cert. but do carry the original one for verification along with other docs.
