I m not sure of distance certificate as for me address proof was enough last year. For first child you need to get an affidavit from notary wherein the school generaly provides a format or else you can mention the requirement and the notary prepares it for you .

Do I have to prepare the affidavit for every school or photocopy will work?

Photocopy should work . Once selected originals can be submitted.

Does it really say certificate? Some schools ask for print of distance measurement of home from school as a proof to validate the distance. That can be checked through Google map by measuring aerial distance between two places. Bal Bharti school provides the description how to do that or you can even Google. You can provide print of this.
