Hi,
On my daughter birth certificate my wife name is mentioned as
Neha Sareen (my surname)
Where as in every other documents she has is of Neha Agarwal. (her maiden name)
So we are filling all school forms with her Maiden name.
But two schools pointed that out while submitting the form, and made a correction in her name as Neha Sareen (as in birth certificate)
Is it a big problem if mother name is not exact match in birth certificate and form.

Yes it can cause problem, better attach affidavit stating same along with B.C.
Some schools might create issue, but not reject. To be on safer side either email them or submit physically.
Yes, if you can do it after form submission better ,else after admission is shortlisted.
Notary attested affidavit will work.

You can give copy of you Aadhaar and mention that in forms you have given your maiden name.No issues.
Better send copy of Aadhaar now to be on safer side.
