Its not mandatory , very few  school want you print the name of school .Get 5-10 common girl child affidavit handy.

same i also wanted to know whether first child affidavit required by schools need to have name of the school in the affidavit also or a general affidiavit with no mention of any school is acceptable to schools.

Seperate for each in original, rest what ever points school is giving ( ex- girl child/single parent) you need to give affidavit.

No, Its common ..1-2 two schools might ask you to print the name of school.
