MOUNT CARMEL SCHOOL, Dwarka

This is the cheating verdict of the management

the age criteria in Mount carmel dwarka is given as 3+ as on 30th Sep 2012. isnt it against the rule of 3+ as on 31st Mar 2013. My son is Jan 2010 born, does it mean he will not be eligible there

Which one is correct ; 5Km or 6km? This is only girls school correct?

Reema ji, it is co-education school both for boys & girls.

And as per respective school offices the distance criteria is correct i.e. 6 km for Nursery class at Anand Niketan branch, & 5 km for Dwarka branch of the Mount Carmel school

1. Registration
i) The admission process has been carefully designed to provide equal opportunity to all applicants and to maintain
transparency in the admission procedure.
ii) Registration Forms for admission in General category can be downloaded from the school website
(www.mpsshalimarbagh.com) . Downloaded forms duly filled up can be submitted at the school counter on
payment of requisite charges on all working days from 1st January, 2013 to 15th January, 2013 between
8.30 a.m. and 12.00noon.
iii) Registration Forms for EWS category will be available in the school on all working days from
1st January, 2013 to 15th January, 2013 between 8.30 a.m. and 12.00noon.
iv) Incomplete forms will be rejected.

7. Documents to be submitted at the time of Registration.
The attestation should be done by gazetted officer
1. Birth Certificate issued by the MCD / appropriate authority. Name of the child must be endorsed in the Birth Certificate.
2. Medical Fitness Certificate of the child. (from registered M.B.B.S. Doctor)
3. Residential Proof. (Ration Card / Adhar Card / Election Card / Passport / Last paid Electricity OR Water OR MTNL Bill). In case of rental accommodation, the applicant should submit copy of rent agreement that the applicant has been residing at the given address for minimum six months at the time of the submission of the form.
4. Alumni (Passing Certificate of having passed Class XII from this school).
5. Sibling (copy of School Identity Card / Report Card).
6. Transfer Cases (copy of Transfer Order from employer stating date of transfer – effective from April , 2012 onwards)
7. In case the child being the first born, affidavit stating the same has to be given by the parents. (Format supplied with the Registration Form)
8. Additional documents for EWS Category :
Submit anyone of the following documents as proof :
Date of birth
i) Birth certificate under the Births, Deaths and Marriages Certification Act, 1886.
ii) Hospital / Auxiliary Nurse and Midwife (ANM) register record.
iii) Anganwadi Record.
iv) Declaration of the age of the child by the parent or guardian.

Present Residential Address
i) Ration card issued in the name of parents (mother / father having name of the child)
ii) Domicile certificate of child or of his / her parents.
iii) Voter I-card of any of the parents.
iv) Electricity bill / MTNL telephone bill / water bill.
v) Unique Identity Card of mother / father / child issued by Government of India.
vi) Passport in the name of any of the parents or child.
Proof of Income
i) Income certificate issued by a Revenue Officer not below the rank of Tehsildar
ii) BPL Ration Card (Yellow Coloured)
iii) AAY Ration Card (Pink Coloured)
Proof of Disadvantaged group
i) A Certificate issued by a Revenue Officer not below the rank of Tehsildar or any other Competent Authority.
ii) Caste Certificate issued by DC Office.
iii) Medical Certificate from Govt. Hospital, incase of children with special needs / disabilities.

iv) A certificate issued in accordance with instruction issued by the Department of Woman and Child Development, GNCT of Delhi, vide Order dated 11.12.2012 in case of Orphan Child.

  • · Please fill up this form very carefully as admission will be based on the contents of this form.
  • · Use only black / blue ballpoint pen to fill the form.
  • · The form should be filled by a parent in own handwriting.
  • · Kindly submit the duly completed form to Laxman Public School Fee Office latest by 12.30 pm on 15th Jan 2013
  • · You are advised to keep a photocopy of the filled form.
  • · For any further communication with parents, the school will cite only the Registration Number given on this form
  • · Attach the following photostat documents with the form (i) Certificate of Birth (ii) Proof of Residence (iii) Others as stated above [Section A]
  • · Children who complete 3 yrs as on 31/03/2013 are eligible for admission for the academic session 2013-14
