Admission in Class VI in Sarvodaya Vidyalaya without having a Transfer Certificate

Hello,

I want my little brother to get admission in Sarvodaya Vidyalaya. He completed his education up to 5th class at home. Now in Sarvodaya Vidyalaya they are asking for an affidavit stating that the child completed his/her education at home. But I do not have any idea of the format for this affidavit. If any of you can help, I would be grateful.

You can get it made at any notary near your home .They will also give you the format.

Thanks..Yes, I tried for that. Since they too did not have any idea of the format, that is the reason I was looking for the format on internet.

But finally, I made an affidavit stating :

I,........., S/O...... R/O ........ do hereby solemnly affirm and declare as under:

1. That the name of my son is.........

2. That the exact and correct date of birth of my son is ..... who born at Delhi.

3. That his mother name is ..........

4. That due to financial conditions of my family, I could not register him in any school.

5. That he has completed his studies up to standard V at home.

6. That the above mentioned information is my true and correct statement.

I think this should work...
