hi balwinderji & Bij ji
just need your advice on the documents that needs to be submitted .In the school website they are referring to the Attested photocopy .Whether it is self attested or it needs to be attested by Gazetted officer or whether the attestation of notary wiould suffice .

A child is eligible this year if he/she fulfils the following conditions:-

•The child should be 3 years of age as on March 31st, of the session year

II Eligibility Criteria 3+ as on 31st March 2011

III Date of Distribution of Registration forms
A) At the School Reception (By Hand) 1st January to 15th January 2011
B) On-Line (www.dav-sreshtha.org) 1st January to 15th January 2011

IV Filled-up forms shall be submitted upto 15th January 2011

V Date of display of First List and Second Waiting List ---

LIST OF SUPPORTING DOCUMENTS REQUIRED AT TIME OF SUBMISSION OF FORM (Attested photocopy of no. 4 to 9)
1. Recent Passport size, coloured photograph of the Child, Father and Mother.
2. Proof to validate First Child (Affidavit on Rs.10/- Stamp Paper).
3.Birth Certificate of Child issued by MCD/NDMC. 1.Proof of Residence - Attested Photocopy of Parent(s’) Ration Card, Voter’s Identity Card, Telephone Bill, Electricity Bill, Water Bill, Passport,Gas connection receipt, Driving License, Registered Rent Agreement prior to January 1,2011 (Any two).
2.Proof of Alumni of DAV Sreshtha Vihar i.e. Photocopy of class XII Certificate.
3.Proof to validate status of single parent.
4.Proof to validate child with special medical needs.
5.Proof to validate Sibling Application (copy of last Fee Bill or I-Card or Report Card).
6.Parents' Identity Card issued from the work place (if available)..
