Simply visit the nearest notary and tell them that you need an affidavit for FIRST BORN CHILD. Most notaries have the format stored on their computers. No need to take anybody along with you.

By the way, the format has been given in this forum itself. Use the search feature to track it down.

yes, I have also got a call from them today and they have asked us to meet the vice principal tomorrow. but me and my husband have decided not to go. Firstly, it is not a very good school, secondly, A school that is itself not abiding by rules would never know the importance of its own rules.
