Affidavit for First Born child and Birth Certificate

Hello,
My daughter is 3 years 4 months now and we are looking fwd to her admission to the big school next year. I wanted to ask

1. How does one prepare an affidavit as proof for first born child/girl child.
2. we already have a birth certificate but it does not have my daughter's name as it was made immediately after she was born and naming was done later. Would that mean I would have to get another certificate made with the name on it.

Affidavit:

ON RS 10 STAMP PAPER

I, (FATHER /MOTHER NAME) SON/DAUGHTER OF ( ) R/O (UR RESIDENCE ADDRESS) do hereby solemnly affirm and declare as under:-

1. That I am the Father/Mother of the applicant ( name of child) who is applying for admission in pre school (nursery) academic session -2012-13.

2. That (name of child) is single born child and I have no other childern.
3. I have read and understood the contents of this affadavit which has been drafted by me/under my instructions and I say that the contents therefore are true and correct to my knowledge.

VERIFICATION:-
Verified at New Delhi on this _________day of Dec 2011 that the contents of this affadavit of aforegoing are true and correct to my knowledge that no part of it is false and nothing material has been concealed therefrom.

Regarding Birth certificate you can include the name from MCD office and it would take 15 days to get BC with name. Some schools are specific about name and many just ask for birth certificate. Better have it done.

Approach the MCD office nearby your place along with
1) A copy of original Birth certificate
2) 2 copies of application for name addition (out of which 1 is for your receiving)
3) Affidavit on Rs. 10 Stamp Paper (Attested by Notary)
4) Address Proof

They will give you date (of approx after 15 days). Go on that date to MCD office again & get the Birth Certificate. Rs. 20 is charged per copy & Rs. 10 for Name Addition. (eg. for 5 copies, Rs. 110 will be charged)
