Affidavit for nursery admission

if we are applying for 10 schools do we need to make 10 affidavit or do we need to create just one affidavit and submit the copy of it to each school?

A voluntarily made declaration in writing, signed by the affiant (person making the affidavit) and accompanied by an oath (regarding the authenticity of the contents) is known as an affidavit.

So affidavit should be original. You have to make 10 affidavits for 10 different schools with if applies and have 2 or 3 extra at the time of admission if you get admission in more than 1 school. While submitting application they you have to submit original affidavits too.

Does school needs school name printed on the affidavid ?

It's school specific and only 1 or 2 schools ask to mention and if so make it later on. So make in a general format which can be used for any school and cost effective too.
