This is with respect to my daughter Dityaa karole nursery admission 2019-2020 form submission issue. While filling up the form by mistake surname/last name is mentioned twice with first name block as well in the last name/surname box.

Can any one how can i correct that as ihad already sent mail to school for this correction but no response. 

Should i submit the new form ?
