Nursery Admissions in Delhi NCR 2026-27

Birth certificate

In my ward birth certificate fathers surname is missing but 1st name and middle name is correctly written.while filling the form or making any affidevit whether I should write full name with surname or write only the name printed on birth certificate of child i.e without surname of father.
Plz help and give suggestion
