Content of Medical Fitness Certificate

Can any one tell what should be the content of Medical Fitness Certificate for nursery admission.

What about Immunization Record?  Do we have to take a copy of Immunisation Chart and attach or should we have to get it printed from doctor in origional.

Content is here but doctors know how to write in their letter head
