Form Dates | Admission Criteria | Results | Fee Details | List of All Schools
Hello,
I see quite a few schools listing the requirements for ATTESTED documents to be submitted along with the forms. My question is: Who can attest the documents? Do they have to be attested by a Gazetted Officer? More specifically, are documents attested by Chief Managers/AGM of Public Sector Banks acceptable?
Another requirement for some schools is the medical certificate/immunization record. Do we need to provide original medical certificate or copies (attested?) of the medical certificates to each school?
Thanks a lot for the help!

You have to provide original medical certificate if school ask for the same and a registered Govt. Doctor will write in their letter head and give you. Immunization record will be asked by few schools and it can be photocopy or attested photocopy.
Every qualified doctor must register with the respective council like Medical Council of India & Central Council of Indian Medicine/Homeopathy/Dept. of AYUSH,MOHFW, GOI. You can get from private Doctor or Govt. doctor but that doctor should be registered and have registration no. or id.
Hi does that mean if 10 schools for which I will be filling up the forms ask for medical certificate I will have to get 10 medical certificates made by the doctor???? Do you think any doctor would give 10 copies of the certificate....
@rachel, to my knowledge, only one will be sufficient, you need to submit only photocopies of the same. Your form cannot be rejected with photocopy of any document. If it does so, immediately lodge complain at 1031.
Dear Manu Sehgal,
As per GOI, Ministry of Personnel, Public Grievances and Pensions Memorandum No. K.11022/67/2012-AR dated 10 May 2013, some Ministeries/State Governments have adopted the provision of self certification of documents like marksheets, birth certificates etc by the applicant/stakeholders instead of asking for an attested copy by a Gazetted Officer or filing of affidavits. However, for verification of the same, original documents are to be produced.
Therefore, no need to get copies attested by notary/G.Os.
Dear Mr. Tanwar,
Thank You for the response!
Does the education sector fall under the purview of the same memorandum? From what I read above, it may be applicable to Ministry of Personnel but I am not sure if DoE would adopt the same.
Typically the statement would read "self-attested copies" if those are acceptable. In this case, the statement only says "attested copies" - I am sure no parent would want to run the risk of application getting rejected for the want of an attestation!
Thanks,
Dear Manu,
I felt it necessary to make it public information. If parents are not ready to adopt the procedure/facility, let it be. Anyways, my children are in jobs. All the best to everyone.
