A bit complicated for address proof. Better to get in writing from your department. Better to take Aadhar, passport, DL, voter card or any other document of same category

Take passport or voter id for addess proof for both the parents... 

Carry the medical form and immunisation card photocopy of the child

Attested photocopies along with original documents have to be taken to DPS Mathura Road. Photocopies have to be attested by whom ??

There are around 3800 candidates, i think we have less chances for admission.

Plz check the serial nos having some gap not in sequence having major mismatch, so we couldn't estimate no of entries
