and it is mentioned on the top of the page that it is for 2013-14.

But please tell me..if the DoE is saying that the admissions will start from 16th..then why is this different..

Hi plz guid m at d tym of taking form wat all docs school wud require & Birth Certificate copy shd b attested by whom?

Generally you no need anything expect Rs.25 cash. But few school in case this school Faith Academy demanded for "Kindly bring the original Birth Certificate as well as the attested copy of the birth certificate at the time of purchases of forms."

Lathaa,at the time of submitting the forms at school,will the parents get a receipt?

Most schools give receipt. But if schools charge more than Rs.25 make sure you get a receipt for it.
