First Born Child Affidavit

Do we need to submit the original affidavit for first born child to each school or we can apply xerox copy of the same will suffice the purpose?

Go to any Notary.... They all have the format ready. Its very common so they all have that. I got it made today and it was pretty standard. He just inserted Names & Address.
