Thanks Sakshi for the reply. That answers one of my questions too. 

But my kid is a Girl & a First Child. Can we combine these two affidavits? Do you have a sample Affidavit?

Instead of first child just add first girl child in the format.

Under the DG category, only Delhi Originate people is allowed or anyone from other states but staying in delhi is also eligible??

As Under the DG Proof list its mentioned that  "Caste Certificate by Revenue Dept GNCT Delhi" need to provide. Can I submit caste certificate issued by other state govt.

is there any specific template for first girl child affidavit or I can use any online available template.

also this needs to be on notarized or self declared on plan paper?

Self declaration can be on a plain paper if schools ask for it.

You  girl can that your child is the first girl child in the affidavit which is notarized.
