First Child Affidavit

The documents required on your page mentions about the First born affidavit on a non-judicial paper of Rs.10/- hence i believe that one can get that made even from notary. But there are a few schools that do not accept the same. They want the certificate signed by the SDM. Please suggest if notary certificate is actually not applicable? pasting frank anthony's link as one of the examples.

Thanks Money. For address proof they have mentioned just ration card & voter ID card. We have rent agreement, telephone bill (airtel) and aadhar card only. Does that mean we will not be able to apply in Frank Anthony Public School?

I was counting a lot on this school as it is just 1.5 km from where we stay.
