Hi,
In many school there is point for first child girl ...so they demand for certificate...how will we get that certificate?

Refer the format of affidavit and self declaration for the same here

Hello,

Refer to any of your nearby Notary, they have set formats available. Get it done on a  ₹10 affadavit, they will charge some nominal amount as their fees. 

Get 2-3 originals made, it will help you while blocking the seat after 1st list.
