Last name not in birth certificate

Hi ,

Is it manadtory to have last name on birth certificate?

Our child's last name is not printed on birth certificate and we wish to add last name while applying for nursery admission. Will this have any impact on admission form acceptance or selection procedure?

Please clarify our confusion. Thanks in advance
