What needs to be filled in Immunization Status for masonic public school?

and is the medical fitness certificate mandatory for all students to be submitted at the time for form submission for masonic? if yes, any format?

Immunization card details are asked by all schools and medical certificate any of your family doctor or any doctor on letterhead will issue .They know the format.

Rest if any school specific format then fill according to that.
