These two questions were not in my form II. 

How do you make sure that your child eats a balanced and adequate healthy diet?

Define you child with tick 10 point out of 14 point.

It had some questions like

How often you tell your child stories and how does it help?

does your child share your toys with other children?

if your child is insisting on watching late night cartoon show, what will you do?

how much time your child watch TV?

Did I miss something or they are using different sets? My form was 4 pages(including parents details).

They have multiple sets. Each containing different set of questions. Even ours was same as yours.

Rikesh,School Specific Dec onwards you can check the website and all new /International school are doing along with nursery.

Esha, Every year they come around 10 Sep (so it can be anytime around that)

I would like to know the feedback about The Millennium School (Sector 41) , i believe it is different from the Shri Ram Millennium Sec 135.

Sumit,That is run by educomp and is equally good.only Shriram name is not there.
