Documents to be attached at the time of submission of forms:

• Recent passport size colour photograph of the child, father and mother, duly pasted on specified place marked on the admission form.
• Attested photocopy of Municipal Birth Certificate.
• Attested photocopy of residence proof.
• Document of Girl/ First Child.
• Document of Inter state transferable job.
• Document of Social/ Noble cause.
• Document related to income criteria in case of student belonging to EWS/ Disadvantage Group.
• Any document related to the admission (as per point system/ Department of education).

*Original documents to be produced at the time of admission. The parents will be responsible for all entries in the Registration Form vis a vis its authenticity. If false entries are detected on verification at anytime, the same will lead to disqualification of the candidature, even if admitted

Schedule for admission

• Date of distribution of registration forms: 20th Dec’14 (Saturday) to 7th Jan’15 (Wednesday)
• Last day of acceptance of duly filled forms: 7th January’15, Wednesday
• Display of lists ( 1st): 24th January’15, Saturday
• Last day of fees submission : 31st January’15, Saturday
• Display of lists ( 2nd) : 4th February’15, Wednesday
• Last day of fees submission : 7th February’15, Saturday

Registration forms will be available as per the schedule at the school.
Registration forms received after 12 noon on Wednesday, 7thJanuary’15 will not be accepted.

1.dpsvv &dps eok-Do we have to physically submit form. But what I understand from the site is we have to submit if shortlisted???
2.dpsvk, is asking to send DD n form by POST.please advise if can submit in the school physically??Thanks..pls advise if somebody has already filled..totally confused

i have filled the form online today and have received reg no too....the site specifies only online submission for general category...i did not have any problem...please let me know if there is a hitch

Is it a must to send the DD by post only? Or can we submit it along with the Regn form at school.

Also, is there any specific format for First/Girl Child affidavit?

The Age Limit for admission in Class Pre School (Nursery) is 3 to 4 Years.
The Registration for admission to this class is open in the month of December every year.
The admission to all other classes depends upon the availability of seats every year and Registration is also open at that time.

CRITERIA FOR ADMISISON IN PRE-SCHOOL (ENTRY LEVEL)
The admission to Open seats (General Category) shall be made only on the basis of fixed parameters and points as under:

The1st list of students selected for admission will be displayed on 24th February 2015. You are then supposed to complete the formalities of admission to confirm your seat. In case, your child is not selected in 1st list you should wait for the next list to be out (in any).

Admission Procedure 2015-2016

Parents have to apply personally at school reception.

Points to remember:

All the candidates should register themselves between 20th December, 2014 to 08th January, 2015.
All the registration forms along with necessary documents should be submitted along with the registration form at the school reception between 09:00 a.m. to 12:30 p.m.
The Registration fee for admission is Rs. 25/- for general category.
Registration is no guarantee for Admission.

Documents required at the time of registration / admission:

Attested photocopy of Date of Birth certificate of the child issued by M.C.D. or equivalent authority.
Attested photocopy of Residence proof - Electricity bill / Telephone bill / DJB bill / Voter ID / Passport / Lease Deed.
Post card size photograph of parent along with the child in centre.
Proof of Sibling (wherever applicable).
Proof of Alumni (wherever applicable).
Proof of Interstate Transfer case (Wherever Applicable).
Latest Original Medical Certificate of the child duly signed by the parent.
Attested photocopy of SC/ST/OBC Certificate (wherever applicable).
Attested photocopy of Ration card (BPL) in case of E.W.S. Quota
Attested photocopy of Income Certificate from S.D.M. in case of E.W.S. quota.
S.L.C. / T.C. from the last school attended duly counter signed by the competent authority (wherever applicable).
Attested photocopy of previous class report card of the child (wherever applicable).
