No. For fitness certificate a registered Doctor or your paediatrician will write and give on their letter head.

No nursery and entry level is class I.

Sakshi it means we have to have birth certificate( with name in it) in original in abundance to apply in schools and secondly do we need to give medical fitness certificate in original to every school while applying??Because my doctor will give only one cerficate.

For applying in schools just a copy of original BC is enough. But for admission and verification we need original documents since each school may give different dates for admission. So have few copies of original BC ready. Medical Fitness certificate is school specific and you can even go to any govt. registered doctors in Primary Health centres or near by Govt. Hospitals if your doctor does not give more than one.

Court registered rental lease dead at least 6 months old is valid.
