Hi, would anyone know if both parents are needed for DPS mathura road documents verification or is one parent enough? Thanks

Just one parent is fine. My husband went alone to dps mathura road today and I went for vasant valley verification which was at the same time. Just a reminder that they are insisting on colour printout of the form. Incase you can't manage attestation by gazetted officer even self attested is fine.

thanks this is very useful.

One more question regarding medical certificate. In their format their is one certificate regarding immunisation from the immunologist who vaccinated my child. Unfortunately, they are not available since my child was vaccinated in Kolkata. Any ideas would be grateful???? 

My date is on tuesday. Thanks and appreciation in advance

Hi Niladri. We also had an issue with the medical certificate as our pediatrician is travelling. The authorities at dps mathura road said that the completed medical certificate can be given at the time of admission. Could your pediatrician vouch for and sign on the immunisation record? Good luck!

Thanks for your inputs mam.

Actually 2 of her immunisations were done in kolkata and one is delhi. The one in delhi is travelling right now. Wont be possible to get any certifications from Kolkata.

I think I'll try my luck in convincing them that the delhi one can vouch for her immunisation 'based on records available' . My immunisation chart is signed. Lets hope for the best.
