Hi Sakshi,

I am filling form for St. Francis. Any ideas if last name of mother matters. In birth certificate of my daughter it's last name after marriage but in my other documents it's my maiden name. Can I use my maiden name in registration form or will it cause problems?

Give the name that you will submit in school for address proof etc better give the after marriage name n in case of any issue you can give affidavit. no worries

All my other proofs have my maiden name. Only in birth certificate of my daughter it's the last name after marriage, so I thought of putting my maiden name.

No worries if you dont have any option ,in case of any issue you can give affidavit (in case it is required)

Hi Monika,

same is the case with me so I have decided to go with last name after marriage where school is not asking for pan card/salary proof as that contains my maiden name :). as Sakshi said we can give affidavit if there is a conflict.

Thanks Chayanika. I had put my maiden name but at the time of submission they said we will consider what is in the birth certificate so they corrected it.
