Please Note: Incomplete forms will be rejected.
1 Paste the photograph of the child on the print out
2 Select the class NURSERY
3 Type the name of the child in capital letters.
4 Select the Date, Month and Year of Birth of the child
5 Tick the gender of the child whether Male or Female.
6 The age of the child (as on 31st March 2012) will come automatically in the system. .
7 Type the name of the father in the given space.
8 Select father's qualification from drop down list.
9a Select father's occupation
i. In case of Govt./Semi Govt servant – Proof of Designation is required.
ii. In case of employee of public/private ltd companies (a) proof for designation (b) no. of employees in organization is required.
iii. In case of owner of public/private Ltd. companies, proof for (a) No. of employees in Organisation (b) No. of Assistant /Junior Engineers in case of Contractors.
9b Select Yes or No whether the job is transferrable or not.
9c Select the designation of the father.
10 Type the Company/ Institution/ Organisation's Name & Address, phone number and pincode in the given box.
11 Type the Home Address, mobile/phone no. and pincode in the given box.
12 Type the E-mail address .
13 Type the name of the mother in the text box
14 Select mother's qualification.

21b Is the child suffering from any disease like:- Diabetes, Asthama, Epilepsy, Heart Disease, Communicable Disease, Speech ,PhysicalDisability. If yes, Give details.
22 Select the neighbourhood area from school to the house and system will automatically identify the distance in kilometers.
23 If any parent is an alumni of the school then enter the year in which he/she passed out.
24 Type the name and class of sibling (Real brother or sister) studying in Gyan Bharati Saket.
25 In case of single parent, tick yes or no.
26 Tick Yes if child is an only child-Girl
27 Tick Yes if child is First born child (Parents are not alumni of Gyan Bharati School)
28 Tick Yes if parents of the child work in military / paramilitary forces
29 The name of father/mother/gaurdian and the name of the child in the undertaking will come automatically in the system.
30 Paste the passport size photograph of the father on the print out
31 Paste the passport size photograph of the mother on the print out.
At the end, submitted date and registration no. will come automatically when the form is submitted by parent or guardian.
Please submit a signed print out of the form submitted online and attested photocopies of the following documents along with registration fees (Rs 25 only), by 16th January 2012 upto 1.00 pm at the school reception.

a) Birth Certificate issued by the Municipal Corporation.
b) Proof of residential address [Passport/Lease Deed/Electricity Bill/MTNL Telephone Bill/Electoral Card].
Note: School will also verify the residential address through home visits.
c) In case of sibling applicants : Admission No. and fee bill issued by the school.
d) In case of Alumni(father/mother/both): Admission No., Year of Admission/Passing, Roll No. of CBSE Class XII Board Exams.
e) For proof of Parents Qualification: Highest Educational Degrees.
f) For Single parent-Proof of marital status should be enclosed.
g) In case of Single Child (Girl) : Attested Affidavit.
h) In case of First Born Child (Parents not Alumni): Attested Affidavit.
i) For proof of Service: Identity Card / certificate issued from the concerned Departmental Head.
NOTE: The registration form is liable to be rejected if the above mentioned documents are not enclosed.
Final admission is subject to the verification of all the documents (in original).

Mother's International School..as of last year this year also stresses on its 3 main criteria: Neighourhood(max40 pts.), Sibling (30 pts.) & Alumni (30 pts.
Being this a very old & reputed school with strong Alumni base there have been rare chances for anybody other than Sibling & Alumni qualifying in it.
Last year was there a golden chance for some aspirants in pre school&pre primary where a lotter was done for a few seats amongst many 40 pts. candidates who were residing in the nearest vicinity of the school i.e. IIT&NCERT campus, sarvodaya Enclave etc..and a lucky amongst them went through..
This is an eye opener for we parents who want to put our kids in these reputed brands but then there is no or rare chances if we are non-alumni & non-sibling cases...so schools like MIS, Springdales,DPS Rohini always give a setback for such aspirants.
It is better to try for newer schools where alumni & sibling criteria is not pushed for that much..
