Since we are talking about two different locations, I would like to believe yu would need to fill 2 different forms. 

However dont count too much on the JP branch since it doesnt have a lot of space to expand more.

Only one you have to fill and forms can be download.

How about distance criteria? What is their base location? Vikas Nagar or B2 Janak Puri?

good point raised jaspreet sir really give us the information on this i am living in hari nagar will i be eligible for vikas nagar as already in my neighbourhood student is studying in vikas nagar but he is elder my daughter is 4+

School never came with point criteria but just registration and so it is upto school management.

pls tell me that medical certificateis to be printed on plain paper or doctor's letter head.

I just got my medical certificate made... the matter was written by the doc on his letterhead...

Yes, its the doc who will write it on his letterhead. He will primarily charge for that only :)

Registration process and Schedule for admission to Pre- School ( LKG) will be as follows:

* Commencement of Admission Process 01-01-2013
* Commencement of Availability of Admission Application 01-01-2013
* Last date of submission of admission applications in school 15-01-2013
* Last date for displaying the list of selected children including waiting list 15-02-2013
* Last date for displaying the 2nd list (if any) 28-02-2013

* Closure of admission process 30-03-2013

Questions regarding Medical fitness certificate (not immunization chart):
1) Is it typically asked for at the time of form submission, or at the time of admission once child gets selected?

2) In one of the threads, I read about having multiple copies of these from the doctor. Our pediatrician mentioned he provides only 1 certificate and parents can get photocopies? Is this fine?

Rohit , it is asked only by the schools where your child gets selected and has to be in original, But you can keep attested copies in case your child get selected in more than one.
