Many Thanks Lathaa.....yes, i will make an affidivity and send it to MCD for the name...once more query....as it might take time, can i submit affidivit or my son passport in case they ask for the name on DOB

CAN ANYONE HELP ME WITH DPS VASANT KUNJ FORM?? Under the column Single parent, I have to indicate that it N.A., but its very unclear where to click. Pls help!!

@ Puja sure but check for second saturday many schools will remain close. Don't rely 2nd saturday.

6. School Photo ID/Any fee bill copy for sibling cases
7. One photograph each of Father, Mother, Child
(b) EWS Category
1. Attested copy of Birth Certificate
2. Attested copy of Residence proof
3. Income proof certificate issued by Sub divisional Magistrate
4. SC/ST Certificate if applicable
5. Certificate from orphanage if applicable
6. One set of photograph of Father, Mother and Child.
Original Documents required to be verified at the time of admission.

5. Copy of admission receipt/School photo ID – In Sibling case.
