Only Child Affidavit

Hi please advise can we use the general affidavit of only child(single one for all the schools)

Like can we use the general verbiage that we are applying for various school in delhi for nursery admission and need submit the same to all the schools or do we need to exclusively mention the school name in each of the affidavit while applying to different school.

Common affidavit without school name can be done unless school particularly mention for school name

* This affidavit should be on a non-judicial stamp paper of Rs. 10/- and should be duly attested by Notary Public.

do hereby solemnly affirm and declare as under :

1. That I have applied for admission of my
son/daughter named _______________________________ in Pre-School/ Nursery
(Nursery) (2015-2016).

2. That I declare that __________________ is our first born(Boy /Girl) child / Only child

and he/she does not have any elder sibling.

DEPONENT

VERIFICATION
Verified at Delhi on ________ day of ________ 2014(or 2015) that the contents of my
above affidavit are true and correct to my knowledge and no material facts have
been concealed therein.

Thanks for the update please advise do summer fields school eok, tagore internation school eok, the indian school sadiq nagar, dav public school kailsah hills, required the specific affidavit of general will work nothing is mention clearly in there guidelines.
