Documents to be submitted:
• Self attested copy of Birth certificate issued by the Municipal Corporation
• Vaccination and Medical fitness certificate of the child.
• 02 passport size photograph of the child and 01 each of the parent
• Pay Order of Rs.35,000/- (Rupees thirty five thousand only)in favour of Delhi Public School,
Dwarka as lump sum advance towards school fees. Balance amount will be payable in three
equal installments in July 2012, October 2012 and January 2013.
• Enrolment Form to be filled and signed by the parents (form will be issued by the school on the
day of admission formalities)
Parents are requested to adhere to the timings to avoid inconvenience to other parents.
Parents are also requested to note that non completion of admission formalities on the date specified
will be treated as their unwillingness and they forfeit the claim of admission and the seat would be
offered to the next eligible applicant.

The following documents in original are required to be brought for verification along with a set of
attested photo copies that are to be submitted in school.
1. Original Birth Certificate of the child issued by Municipal Corporation or equivalent
authority.
2. Any two Proofs of residential address (Lease Deed/ latest paid Electricity/ MTNL Bill/
Electoral Card/ Driving License. In case of rental accommodation, valid Rent
Agreement and an affidavit on stamp paper to be submitted). School reserves the right
to verify the residential address through home visits.
3. Self Certification of parents with regard to Girl Child or first born child.
4. Medical fitness certificate of the child issued by Medical Officer.
5. In case of sibling applicants, the school Identity Card /Fee Bill of sibling studying in
DPS Dwarka.
6. In case of applicants with special needs- Certificate of disability issued by the
competent authority.
7. Proof of Transfer to Delhi.- Transfer Order and joining report stating date of transfer
and date of joining.
8. Copy of AISSCE (Class XII) Board Pass certificate of the parent for the claim under
alumni.
9. Provide a valid/ legal proof (Death Certificate) and undertaking of his/her single status
for claim under ‘single parent'.
Self Attested photo copies to be submitted.
No undertaking regarding the documents mentioned above will be accepted at the
time of verification.
The list of selected/eligible applicants for draw of lots as the case may be after the
verification process, will be displayed on 03 Feb 2012 at 4.00 p.m.

Cambridge NFC/Srinivaspuri can be a preferred choice being a value based established school having a balanced approach towards academics and extra curriculars.
