Schools demanding medical fitness certificate

Just wanted to know do we have any form which needs to be filled up by doctor or just doctor prescription where in they can certify the fitness of child.

And most important who all are eligible to certify this certificate ???

Only MBBS doctor or any child specialist.

Please do help to address my concerns.

Hi,

Yes mostly schools are asking for this. MBBS doctor will write on their Letter head and it will work,unless any specific format is given by school.
