Where to get First Child and Girl Child affidavit?

Hello Everyone,

As the admission date is fast approaching, i am collecting all the documents required for the admission. I dont know where to get the First child & Girl child affidavit? 

Do i need to get it from notary or a self affidavit will work?

Hi Kapil. I believe in the current scheme no points are awarded for first child & girl child, so no point in getting an affidavit. But I suggest you to double check

In the required documents section, it is mentioned that we need this certificate!
