Can you pls help me with a sample template medical certificate for admission process.

Some schools are asking for the same.

This will issued by your family doctor or any other doctor on their letter head. They know the format.

Approach any MBBS doctor near you will get it from him. No issue & tention but take kid with you as he may examin the kid.
