Affidavit in case of first-born child, girl child

Hi All, Can anyone let me know the proforma of affidavit for first born girl child. From where can i get this affidavit registered ? How many affidavits should i make ?

Attested photocopy of the affidavit (without the name of the school) is accepted in most of the south delhi schools. Original Affidavit is required at the time of document verification.

Lathaa..in your affidavits tab you have shared the common affidavit with school name written on it..can you please share an affidavit format which is not school specific.

Its the same format,Just few schools ask it to be in school name(remove the school name..all notary have this format)
