First Child Affidavit attested by SDM --Frank Anthony school

Hi Friends,

Frank Anthony school has request to submit the first child affidavit attested from SDM .

I have prepared the affidavit from notary. Will they accept this or not ? 

Kindly guide.

No You have to get it done from SDM only,Your notary will get it done.

1 more point to add, Affidavit (Stamp Paper) should have name of both parents otherwise it will be treated as Single Parent.
