If you have already filled and submitted the form, you need to approach school as duplicate form submitted might  reject the admission.

The School is not responsible despite of various emails and reminders. In the School also they are not allowing me to meet the concerned authority. What to do?

Keep a copy of email and registration number and try after 17 Jan 2017. Also submit hard copy filled with corrections after Jan 17
